Appointments
Please note, Carla is not accepting new patients at this time. The practice
will re-open to new patients in September. Please email here to be placed on
the waiting list and to be notified when the practice re-opens.
The office now has online appointment scheduling available for your
convenience.
I encourage folks to use the online scheduler. I no longer have a receptionist, so when you
call for an appointment, I might have to call you back to schedule the traditional way.
However, if you prefer to make an appointment by phone, call 543-5444 and select option
#2 when the menu begins.
Scheduling an appointment is simple.
Read this first:
Note! You will receive a notification email that you made the appointment. This is almost
instantaneous. If you do not receive this, your either entered your email address incorrectly
or did not complete the appointment form. The most common cause of invalid appointments
is not scrolling down on the very last page and clicking "make appointment". Once you click
there, you will receive a notification email and, later, a confirmation email. If your
appointment is for the same day, you may not receive the second confirmation email.
IF YOU DO NOT RECEIVE A NOTIFICATION EMAIL, YOUR APPOINTMENT IS NOT
VALID! PLEASE CALL OR EMAIL ME IF IN DOUBT.

To schedule an appointment:
-If you have used the online scheduler previously, then click here. You can log in and then select your
provider and appointment type and time. If you have forgotten your username or password, the system can
recover that for you.
-If you have not used the scheduler previously, or do not remember your login
information, then click on the most appropriate visit type from the list below. The link will
allow you to create a new log in and then you can simply follow the directions from there.
Please remember your username and password for future use.
Appointments are available Tuesday through Friday.
Fifteen Minute appointments
These slots are for patients with one very simple problem or issue. Examples are:
medication renewals that do not require exam or discussion about dosage changes,
completion of forms that do not require an exam, or a problem such as mole check.
Half-hour appointments
These slots should be used for simple problems for new or returning patients. Some
examples are a cold or sinus infection, bladder infections, follow-up on medications,
medication renewals, foot pain or a skin rash. Annual exams for those under age 35 are
also appropriate. If you have more than 1-2 simple problems or have problems to discuss at
your annual exam, please schedule in a longer appointment slot.
Three quarter hour appointments
These slots should used for those with more complicated problems to review and discuss or
if there are more than 2 simple problems. Also appropriate for annual exams for those 35
and older IF I have seen you previously.
One hour appointments
These slots are for the patient consulting regarding a complex problem or if this is your first
visit with us and you have a complex history. Hormones,menopause issues, diabetes,
depression and anxiety are considered complex problems. Also appropriate for a NEW
patient OVER the age of 35 who would like an annual/ preventive exam.
New patients are asked to provide credit card information. We have many "no shows" and we ask that you
provide credit card information per our cancellation policy: If you do not notify us of cancellation or the need
to reschedule at least 24 hours before your appointment, you will be charged a $50 "no show" fee. This
does not apply to appointments made for "same day" appointments which are made within the 24 hour
time frame. Please call if you have any questions. You are not required to provide credit card information
online but we reserve the right NOT to confirm your appointment if you do not. The website is secure and
your information is always safeguarded.
