If you have not used the scheduler previously, then click on the most appropriate visit type from the list below. The link will allow you to create a new log-in and then you follow the prompts from there. Please revise your username and password to something you will remember for future use.

These slots are for patients with one very simple problem or issue. Examples are: medication renewals that do not require exam or discussion about dosage changes, completion of forms that do not require an exam, or a problem such as mole check, simple cold/sinus infection, bladder infection, or other minor illness.
Half-hour appointments
- These slots should be used for simple problems for new or returning patients. If there are 3
or more simple problems, or a complex problem, please schedule a 45 minute visit. - Annual exams for those under age 35 are also appropriate.
- Annual exams for those over age 35 when there are no "problems" to discuss.
Three quarter hour appointments
- These slots should used for established patients with more complicated problems to review and discuss.
- For visits where there are more than 2 simple problems.
- Also appropriate for new patients over 35 for an annual exam. If there are problems to
discuss at your annual, please schedule for an hour appointment. Please be aware that time
does not allow for extensive discussion re: problems and a complete wellness exam.
- These slots are for consults regarding a complex problem or if this is your first visit and you have a complex history. Hormones, menopause issues, diabetes, depression and anxiety are considered complex problems.
- Also appropriate for a NEW patient OVER the age of 35 who would like an annual/preventive exam and has additional problems/issues to discuss. Please be aware that time does not allow for extensive discussion re: problems and a complete wellness exam. Two visits may be needed.
Please take note of the cancellation fee. New patients will be charged a $50 "no show" fee if they do not notify the office of cancellation, or the need to reschedule, at least 24 hours before their appointment. This does not apply to appointments made for "same day" appointments which are made within the 24 hour time frame. Please call if you have any questions. Established patients will be charged the fee after 2 "no shows" in one year.
